The conference will be held at The Resort and Conference Center at Hyannis (508-775-7775), Hyannis, Cape Cod, Massachusetts.
Hotel Accommodations:
A limited block of rooms has been reserved at convention rates ($192.00) at the site hotel (The Resort and Conference Center at Hyannis). These rooms will be assigned on a first request basis. To reserve your room, please call 508-775-7775 and mention the SEAK/Workers’ Compensation Conference. Note: The conference was sold out the last several years. Early registration is highly recommended.
Tuition/Discounts:
Please type or print clearly and make your check payable to SEAK, Inc. Tuition for the Twenty-Eighth Annual Nat’l Workers’ Compensation and Occupational Medicine Conference is $895. This includes handout materials to be distributed at the door, coffee breaks, reception, three continental breakfasts, a social event on Wednesday night, and one conference luncheon. A 20% discount is available for two or more individuals registering together prior to May 15, 2008. To receive the discount, please sign up via phone, fax, or mail (not online). Tuition for each preconference is $395.

Upon receipt of your registration form and payment we will mail you a confirmation letter and driving directions.

Cancellations:
Cancellations received in writing before July 1, 2008 will receive a full refund. Persons canceling after July 1, 2008 will not receive a refund, but will be provided with the handout materials.
