REGISTRATION INFORMATION

How to Register

Simply print the fax registration form, complete it, then fax it to SEAK at (508) 540-8304 or mail it to SEAK at PO Box 729, Falmouth, MA 02541 with a check or credit card number enclosed.

Hotel Rates/Reservations

SEAK, Inc. has reserved a limited number of rooms at special rates ($69.00, $89.00 for concierge level - normal rack rate is $125) at the site hotel, The Radisson University Hotel, Orlando. Reservations are made by attendees directly with the site hotel by calling (407) 658-9008 and asking for the SEAK group rate.

All programs will take place at the Radisson University Hotel Orlando which is located in beautiful East Orlando on the corner of Alafaya Trail and Highway 50 (1724 Alafaya Trail). It is easily accessible to Orlando International Airport, local attractions and minutes away from Orlando's top corporations located in Central Florida Research Park. The University of Central Florida Campus is less than 2 miles down the road. The Radisson has a fitness center which features state of the art equipment, an outdoor swimming pool, business services and Old Chicago Steakhouse and Grille. They have also added a Radisson Van to better serve their guests in which they provide complimentary transportation within a five-mile radius of the hotel.

Continuing Medical Education (CME) Credit

SEAK, Inc. is accredited by the Accreditation Council for Continuing Medical Education to provide Continuing Medical Education (CME) for physicians. SEAK designates 2004 IME Summit, Malpractice Survival Training For Physicians, and 2004 Medical Witness Summit each for a maximum of 14 Category 1 credits toward the Physician's Recognition Award of the American Medical Association. SEAK designates Law School For Physicians for a maximum of 21.5 Category 1 credits toward the Physician's Recognition Award of the American Medical Association. Each physician should claim only those credits that he/she actually spent in the activity.

Tuition, Meals & Discounts

Tuition is $795 for Malpractice Survival Training For Physicians, $995 for Law School For Physicians, and $1,195 each for 2004 IME Summit and 2004 Medical Witness Summit. Students registering for two or more courses will receive a 20% discount off each course. Tuition includes a valuable seminar reference manual, continental breakfast and lunch each day with faculty, coffee breaks, and a dynamic learning experience.

FREE with Early Registration

All persons registering prior to August 19, 2004 will receive a complimentary copy of the highly acclaimed 65-minute videotape Winning Over the Jury: Techniques for Experts that Work (a $104 value). Space is strictly limited for the seminars so register early.

Cancellations

Cancellations received in writing prior to October 15, 2004 will receive a full refund.
